This form is for people and businesses that owe money to the CRA. It takes about seven minutes and gives us the big picture before we speak - what is owed, what the CRA has already done, and whether anything is coming at you personally.

Approximate figures and dates are fine. Choose Not sure where you do not know - a not sure is useful to us, a guess is not.
Please have a photo or scan of your driver's licence ready to attach at the end.

Tell us early if the CRA has given you a date to respond by, or if you are about to make a payment or sign something the CRA has sent you.

Your answers are confidential. Submitting this form does not make you a client of the firm and does not create a solicitor-client relationship. We will contact you about next steps.

We use your answers only to assess your matter and advise you on next steps. Submitting this form does not create a solicitor-client relationship.

Both means the CRA is pursuing a company and a person - for example the company's GST/HST, and you personally as a director.

A CRA letter saying it is considering assessing you personally usually gives you 30 days to reply. The date printed on it is the one that counts.

Unfiled periods usually have to be brought up to date before the CRA will finalize a payment arrangement. It is better to know now.

An audit or a recent reassessment changes both the amount and the options. Tell us even if it is not finished.

Choose every account you know about. If you are not sure, choose Not sure - we will confirm it from the CRA directly.

A round number is fine. If different accounts have different balances, put the total here and break it down in the next question.

For example: GST/HST approx. $40,000; payroll approx. $25,000; personal income tax approx. $12,000.

For example 2021-2024. An approximate range is fine.

Include anything that has already stopped. A garnishment that ran for two months last year still matters.
